*Membership Terms and Conditions Applicable to Evergreen and Sprout Membership for Kids
- Types of Member Cards and Validity Periods
1.1 ) The details and validity period of each member card are as follows:
• Evergreen Peak Summit: Priced at 150,000 THB and valid for 3 years from the activation date.
• Evergreen Upland: Priced at 100,000 THB and valid for 2 years from the activation date.
• Evergreen Forestland: Priced at 50,000 THB and valid for 1 year from the activation date.
• Sprout Membership for Kids: Priced at 35,000 THB and valid for 3 years from the activation date.
1.2 ) All members must activate their cards within 90 days of payment. If the card is not activated within this timeframe, the hospital reserves the right to automatically activate the card on the 90th day, marking the start of the validity period. - Personal Privileges and Verification
2.1 ) Members are required to present their member cards to our staff at the start of every hospital visit.
Each member card is issued exclusively to the person it was intended for, and cannot be transferred, assigned, or given to another person, except in the event of the member’s death. In such cases, membership rights may be transferred to only one immediate family member or legal heir — namely, the father, mother, siblings, legal spouse, or legitimate children. The transferee must present the member’s death certificate, a document verifying the familial relationship to the deceased, a court-appointed estate administrator document, and the estate administrator's letter requesting card membership transfer, per the hospital’s terms and conditions.
2.1 ) Only the room vouchers (worth 3,000 THB each) can change hands, be reassigned, or given to another person. - Usage Restrictions and Refund Policy
3.1 ) The privileges and benefits cannot be changed, refunded, redeemed for cash, or combined with other discounts, promotions, or offers, under any circumstance.
3.2 ) In the event of membership cancellation, the hospital reserves the right not to refund the membership fee, including the value of any unused benefits or services, regardless of the circumstances. - Membership Benefits and Claims with Insurance or Contract Companies
4.1 ) The membership fee cannot be claimed from life, accident, and health insurance companies or the hospital’s business partners.
4.2 ) Members are not permitted to use their membership privileges in conjunction with medical expenses reimbursed by contract companies or insurance providers. However, membership discounts are applicable toward excess charges not covered by the contract companies or insurers. - Benefits granted to members depend on the type of card and are of three categories as follows:
5.1 ) The Voucher and Checkup Program includes health screening programs and other benefits according to the hospital’s terms and conditions for each card type.
5.2 ) The Member Discount Program includes discounts that are effective immediately upon card activation, and discounting privileges for the inpatient (IPD) service that are effective 7 days after card activation, subject to the hospital’s terms and conditions.
5.3 ) The Member Exclusive Privileges: Only holders of the Evergreen Peak Summit and Evergreen Upland cards are entitled to a 5% discount when purchasing any of the four member card types for their immediate family members (limited to father, mother, siblings, legal spouse, and legitimate children). Please note that holders of Evergreen Forestland and Sprout Membership for Kids cards are not eligible for this discount benefit. - Membership Renewal
Membership Renewal must be completed within 30 days of the card’s expiration date to ensure uninterrupted access to discounts and benefits. Renewals made after this period will require a new application, similar to a new member. For inpatient (IPD) services, benefits can be used 7 days after the activation date. - Membership Application
7.1 ) Membership applications for all card types are exclusively available to Thai nationals. Foreign nationals must hold residency status in Thailand (expats). The hospital reserves the right to evaluate and approve applications from foreign nationals on a case-by-case basis.
7.2 ) Sprout Membership for Kids is reserved for children aged from 1 day up to 14 years and 11 months.
7.3 ) Members can conveniently view their member card details and track the usage of their benefits through the My MedPark application. - Maximum Estimated Value of Benefits
8.1 ) The estimated maximum value of benefits a member may receive is approximately THB 617,000 for the Evergreen Peak Summit card, THB 357,000 for the Evergreen Upland card, THB 150,000 for the Evergreen Forestland card, and THB 92,500 for the Sprout Membership for Kids card. These figures are for illustrative purposes only; actual benefits vary based on the decision to purchase additional offers during the membership validity period. Estimates are based on regular prices of items included in the Voucher and Checkup Program (which are higher than promotional package prices occasionally offered by the hospital). The estimated value of discounts under the Member Discount Program is based on maximum usage, which requires additional purchases, as shown in the image above.
8.2 ) Upon expiration of the member card, the cardholder shall no longer be entitled to any card benefits for any reason whatsoever. And shall be precluded from asserting any claims or demands relating to the abovementioned estimated maximum value. - Amendment of Terms and Conditions
The hospital hereby reserves the right to review, amend, modify, or revoke the terms and conditions governing the Evergreen Member Cards, including all associated benefits, at its sole discretion and without prior notice.
