- Click https://medpark.hospital/ModernaRecord for input vaccination record.
- Enter buyer’s ID card number or passport number in the user name field.
- Enter password to login
- For buyer uses Reservation Code as the password. This method will enable you to transfer your vaccination right or modify the name of vaccine recipient and record your Covid-19 vaccination history.
- For the transferee of vaccination’s right uses your date of birth (ddmm) as a four-digit passcode. This method will ONLY enables you to record your Covid-19 vaccination history, you will not be able to transfer the vaccination right or modify the name of vaccine recipient.
Please keep your Reservation Code safe and secure to prevent unauthorized access from others who could be able to modify the name of vaccine recipient with your code.
3. Record the vaccination history
- Once you have accessed the system you will see the detail of your vaccine reservation. Click the green button to check your personal information and proceed to the next step.
- The system will bring you to the Registration Information page, click the blue button to edit/record vaccination history for your safety, please input all of your past vaccination details.
- Check the accuracy of the data and make sure the information is correct and complete.
- If you have received Covid-19 vaccine before input your vaccination history and click save.
- If you have received more than 1 dose, select 2nd dose to input the detail of your vaccination history and click SAVE again.
- The system will bring you to the Registration Information page again. Please recheck your information and click Logout if all information is correct and complete.
4. Transfer the right or modify the name of vaccine recipient
- Once you have accessed the system you will see the detail of your vaccine reservation. Click the green button at the row of the vaccine you'd like to transfer the right or modify the name of vaccine recipient.
- The system will bring you to the Registration Information page, Click the yellow button to transfer/change the name of vaccine recipient.
- The system will request you to enter the Reservation Code again to edit the information.
- The system will bring you to the Covid-19 vaccination right transfer page, click the Reset Personal Data button to delete all data.
- After that, you’ll be able to edit personal information, ID card number, and vaccination history. Once you have finished editing click submit to save the data.
* For vaccine recipient’s safety, Please input all past vaccination details
The adjustment of information, the transfer of right, and the modification of vaccine recipient’s name/the date of vaccination can be done for an unlimited number of times. However, the system will disable the change of information 2 days prior to the vaccination date for the hospital to prepare your vaccination.
If you would like to modify the name or the date of vaccination less than 2 days before the vaccination, there will be an additional cost to unlock the system at 500 baht per time.
* Once the hospital has received your vaccination record
and the vaccines have arrived,
the hospital will start scheduling vaccination appointments
and will inform you of the date and time again
