to register for MODERNA vaccine.

  • The inoculation of MODERNA vaccine must take place at, or by the staff of MedPark Hospital only.
  • In case the hospital cannot fulfill and meet your vaccination reservation, you can request for a full refund.
  • The offered price includes the vaccine cost and hospital service fee on the vaccination date.
  • The doctor consultation fee is not included.
  • The above price does not include medical certificates and other services unrelated to vaccination services, such as health check-ups or doctor visit for different illnesses.
  • The service is exclusively for MODERNA vaccination only.
  • This price is restricted for Thais and foreigners living in Thailand only.
  • The government has allocated gratuitous vaccines to the general public through government-designated channels. Notwithstanding, you still wish to purchase the alternative vaccine to be inoculated at MedPark Hospital and give consent for the Hospital to collect the full cost of this vaccine service and wait for the definite vaccination appointment date.

Remarks

  1. Currently, ongoing studies conducted in the USA and UK regarding MODERNA as the 3rd and the 2nd shot, either with different (heterologous) or the same (homologous) vaccine regimen. The studies result will come out in a year time from the start of the studies in June 2021. Updates will be provided as the results become available. The use of Moderna as a booster or in a heterologous vaccination is not yet a current standard of practice.
  2. The registration for vaccine reservation is intended for 2-dose primary vaccination series (Homologous Prime/Boost) as sanctioned by the vaccine manufacturer. Any future changes will be communicated periodically.
  3. Once the vaccine has been received, the vaccination sequence will be prioritized according to the chronological order of paid registration, and the hospital will send you a personal unique link to book your vaccination date and time in advance. However, your vaccination date will be adjusted according to appropriate recommendations based upon your vaccination history.
  4. Nevertheless, if the hospital could not dispense you the vaccine by the committed dates below, you can request a full refund or wait for the next round of vaccine allocation:
    • Those who reserved for MODERNA vaccine during July 2-4, 2021: April 30, 2022
    • Those who reserved for MODERNA vaccine during August 17-27, 2021: July 31, 2022

  5. The registration for vaccine reservation cannot be canceled, is not refundable and is final.
