Journey to Becoming a Gynecologic Oncologist
Inspired by the National Cancer Institute near her home, she set her sights on pursuing a career in medicine. She initially applied for a scholarship to study internal medicine at the Institute. However, with no positions available, she chose the specialty of pathology instead.
“I studied anatomical pathology at Siriraj Hospital. After graduation, I joined the National Cancer Institute. Yet, after 2–3 years of pathology practice, I realized I wanted to further my studies. It steered me to obstetrics and gynecology at Vajira Hospital, specializes in gynecologic oncology. I believe that by working in this field, I can make a meaningful impact on patients who truly need specialized care. My foundation in pathology provides a solid understanding of how diseases progress, which directly enhances my expertise in gynecologic oncology.”
A Gynecologic Oncologist: One Who Must Profoundly Understand the Intricacies of Women
Gynecologic conditions are often more complex than those in men, as the female reproductive organs are internal and not easily visible. Many abnormalities may develop silently, with patients themselves remaining unaware.
“Gynecologic examination requires delicacy and great care, as it involves intimate areas. Preserving privacy helps patients feel comfortable and less anxious during pelvic examinations. These are things every gynecologist must uphold.”
Female anatomical structures can vary significantly, requiring diagnostic approaches tailored to each patient. Examination options include a standard bimanual pelvic examination, transvaginal or abdominal ultrasonography, or additional investigations as appropriate.
“Today, many women remain hesitant to undergo pelvic examinations. If they have no symptoms disrupting their daily lives, they often avoid screening. This avoidance is concerning because, by the time symptoms appear, it may already be too late for early-stage cancer detection.”
Vaginal Discharge: A Seemingly Ordinary Symptom That May Indicate Something More Serious
A commonplace scenario Dr. Siriwan encounters is women experiencing vaginal discharge who assume it to be routine and delay seeking medical care. Yet, vaginal discharge is often an early warning sign of underlying abnormalities, most notably cervical cancer.
“Chronic or foul-smelling discharge should not be dismissed as a simple infection. Through a pelvic examination, doctors can identify potential abnormalities, detect cancer at an early stage, and initiate timely treatment.”
When Cancer Treatment Brings Renewed Hope
Dr. Siriwan recalls that for many women diagnosed with gynecologic cancer, the news alone can be overwhelming. Many believe survival is impossible, especially in advanced stages. Yet, once they undergo treatment and the surgery is successful, their gratitude is profound.
“Some patients have even bowed down to express their heartfelt gratitude. (I had to quickly lower myself to acknowledge their gesture, feeling respectful and deeply moved by their gesture of profound gratitude.) To them, I was someone who had given them back their life. Seeing that our treatment can genuinely help save lives fills me with immense joy. Caring for cancer patients means attending not only to their physical health but also to their mental well-being. We walk alongside them as companions throughout their battle. Suitable treatment and care options are always available at any stage of the disease. These are why patients should never lose hope.”

“Gynecologic conditions are often complex and intricate. Many women feel embarrassed to undergo examinations or may only seek medical care once symptoms arise. However, identifying the cause early and starting treatment promptly is always better than allowing a condition to progress into a more serious problem. For instance, cervical cancer screening should commence from age 25. Women with a family history of cancer should consult a doctor to develop a tailored plan for screening and preventive care.”
